STAR TREK: VOYAGER “TRANSWARP HUMAN” FROM “THRESHOLD”
In the Star Trek: Voyager episode “Threshold”, “Tom Paris” (Robert Duncan McNeill) made history by becoming the first human to break the Warp 10 barrier however the experience caused him to rapidly evolve and mutate. He escaped sickbay and kidnapped “Captain Janeway” (Kate Mulgrew) where he broke the Warp 10 barrier a second time causing Janeway and himself to both evolve into amphibians – transwarp humans. This prop transwarp human may have been either Paris or Janeway. It is made of latex painted orange with brown along the back, little arms and stubs for legs. It has plastic eyes and one black whisker. The other is lost to time. There is a large slit in the underbelly that contains plastic and tubing, possibly for special effects purposes. Due to time, use and storage, the top of the creature has some damage to the skin. Measures approx. 71 x 18 x 11 inches.

Obviously, one of these pictures is a mistake. ;) It would appear that It's a Wrap! accidently included a picture of Santos' Costume (#3455), a very similar number to this item.
